Breed, Weight, Age, Any Special Needs ( puppy, elderly, meds, anxiety, temperament , dog aggressive etc.) Dates for service, Specific service requesting (ie. Boarding, Daycare, Grooming)
It is also helpful if you refer to our list of required vaccines on the "check list" page. If you do not have all of the required vaccinations, don't worry, we can go over alternatives with you over the phone.
